PUBLIC NOTICE
Request For Proposals (RFP)
Sealed PROPOSALS will be received by
Chubbuck City (hereinafter called the
"CITY") and Chubbuck Development
Authority (herein called the "CDA") at
the Cily Clerk's Office 5160 Yellow-
stone Avenue. Chubbuck. Idaho,. 83202
until 3:00 P.M. local t€me 11 Au ust
2016, and then at said office publicly
opened and read aloud.
Each PROPOSAL must be submitted in a
sealed envelope, addressed to Chub -
buck Citv. Attention: PUBLIC WORKS at
5160 Yellowstone Avenue. Chubbuck.
Idaho. 83202, Each sealed envelope
containing a PROPOSAL must be plain-
ly marked on the outside as PROPOS-
AL for DARK FIBER LEASE and the en-
velope should bear on the outside the
name of the RESPONDANT, his ad-
dress, his license number if applicable,
and the name of the project for which
the PROPOSAL is submitted. It for-
warded by mail, the sealed envelope
containing the PROPOSAL must be en-
closed in another envelope addressed
to the CITY at 5160 Yellowstone Av-
enue, Chubbuck, Idaho 83202.
This RFP includes a multi-year lease
agreement for dark optical fiber strand /
strands for the interconnection of CITY
facilities and provide citizens and busi-
nesses with fiber optic opportunities.

Questions prior to the PROPOSAL sub-
mittal shall be directed via email only to
Rodney Burch, Public Works Director at
rburch@cityofchubbuck.us. No phone
questions will be allowed?
The CITY and CDA reserve the right to
waive any informalities or irregularities
in those PROPOSALS received and to
reject any or all PROPOSALS or to
award or refrain from awarding the con-
tract for work, whichever is deemed to
be in the CITY and CDA's best interest.
Chubbuck City, Idaho, does not discrimi-
nate on the basis of race, color, national
origin, or sex in the award and perfor-
mance of any work covered by this
PROPOSAL.

years of age, and the Principal Clerk of the Idaho
State Journal, a daily newspaper, printed and
published at Pocatello, Bannock County Idaho and
having a general circulation therein.
That the document or notice, a true copy of which
is attached, was published in the said IDAHO

That said paper has been continuously and
uninterruptedly published in said County for a
period of seventy-eight weeks prior to the
publication of said notice of advertisement and is a
newspaper within the meaning of the laws of
Idaho.
