ADVERTISEMENT FOR BIDS
Sealed Bids for the construction of Well
#2 Rehabilitation Improvements will be
received by The City of Chubbuck, at
Chubbuck City Hall, 5160 Yellowstone
Avenue, Chubbuck Idaho, 83202. Bids
will be received until 2:00 p.m. Mountain
Daylight Time on May 2, 2016 at which
time the Bids received will publicly
opened and read.
The Project consists of demolition of the
existing well house and mechanical
equipment and constructing a new well
house with all the associated piping and
appurtenances.
Bids will be received for a single prime
Contract, Bids shall be on a lump sum
and unit price basis as indicated in the
Bid Form. Bids should be clearly
marked on the exterior of the envelope
in which they are submitted with the fol-
lowing information:

Prospective Bidders may examine the
Bidding Documents at the Issuing Office
on Mondays through Fridays between
the hours of 8 A.M. to 5 P.M., and may
obtain copies of the Bidding Documents
from the Issuing Office as described be-
low.
Bidding Documents may be obtained in
three ways: (1) printed sets, (2) on com-
pact disc (as portable document format
(PDF) files), or (3) online via Quest
CDN. Printed copies and CDs are avail-
able from the Issuing Office, during the
hours indicated above. Printed Bidding
Documents may be obtained from the
Issuing Office either via in-person pick-
up or via mail, upon Issuing Office's re-
ceipt of payment for the Bidding Docu-
ments. Documents are available for a
non-refundable charge depending on
document format as follows:
• Printed Sets ($100)
• In PDF format on a compact disc ($30)
• Online at www.kellerassodates.com via
Quest CDN ($20). Click on the 'Current
Projects. Bidding' tab which shows, in
the left hand column, all of the projects
which Keller Associates has bidding at
the present time. Click on the 'Chub -
buck Well #2 Rehabilitation' hyperlink
which will display information specific to
the project.
Partial sets of Bidding Documents will not
be available from the Issuing Office.
Neither Owner nor Engineer will be re-
soonsible for full or partial sets of Bid-

being first duly sworn on oath deposes and says:
that SHE was at all times herein mention a citizen
of the United States of America more than 21
years of age, and the Principal Clerk of the Idaho
State Journal, a daily newspaper, printed and
published at Pocatello, Bannock County Idaho and
having a general circulation therein.
That the document or notice, a true copy of which
is attached, was published in the said IDAHO
STATE JOURNAL, on the following dates, to -

That said paper has been continuously and
uninterruptedly published in said County for a
period of seventy-eight weeks prior to the
publication of said notice of advertisement and is a
newspaper within the meaning of the laws of
Idaho.
