DESIGN REVIEW COMMITTEE MEETING
September 13th, 2005
APPLICANT:

PROJECT TITLE:
Additional Building to east of existing building.
LOCATION:

PURPOSE OF REVIEW:
Site Plan Approval.
Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Larry Kohntopp, Jean Higgins, Ron
Conlin, Ray Griffin and Eric King.
Roger MaIm was present to present the proposed site plan.
Mr. MaIm said the additional building will be assisted living and separate from the hard of
remembering facility. The building will be to the east of the existing building at 4826
Yellowstone Ave.
Staff suggested the facility go to a dumpster rather than several carts. Mr. MaIm stated he will
accommodate the request.
Discussion about the address for the second building. Fire Department requested the address be
4826 Hawthorne with Suite A & B.
Design Review approved the site plan as presented with the following conditions;
1. Keypad entry installed on Suite B.
2. Landscaping with sprinkling, including trees planted.
3. Storm water drainage contained on site. Mr. MaIm will work with City Engineer Sean
Harris.
4. Fire Chief Eric King will work with Roger MaIm to make the 4826 Hawthorne
entrance EMS friendly. Mr. MaIm said he will do what the Fire Department suggests.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the Mayor, Building Official, City
Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required.
The Design Review Team meets every Tuesday morning between the hours of
I 0:30 and II :30 AM.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in
advance of your scheduled meeting. Formal review of your plans or project by City
Staff is conducted after formal application. Once you schedule your Design Review,
it is very important to keep that appointment so others can be scheduled accordingly.
