Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Jean
Higgins, Randy Severe, Ron Conlin, Ray Griffin, Merlin Miller and Eric King.
William Slaven, Outpatient A/Drug Counseling Services was present to present his site plan.
Discussion about required parking spaces.
Larry Kohntopp felt the landscaping is already in place. Mr. Slaven said he would like to keep
the garden that is in front of the home.
Mr. Slaven stated that in the future he would like to add living quarters for approx. 12 people.
Mr. Slaven was told that if the business changes from outpatient to having people stay in
the home he will be required to go through the conditional use permit process.
After discussion, Staff approved the site plan for William Slaven to operate an outpatient A/Drug
Counseling Service at 147 W. Linden Ave, subject to:
1. Landscaping with automatic sprinkling.
2. Meet required parking requirements.
3. Required to install an knox box.
4. Address to be clearly visible from street.
5. Required to get a conditional use permit if other than an outpatient A/Drug Counseling
Service. No patients allowed to stay overnight.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the Mayor, Building Official, City
Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required.
The Design Review Team meets every Tuesday morning between the hours of
10:30 and 11:30 AM.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in
advance of your scheduled meeting. Formal review of your plans or project by City
Staff is conducted after formal application. Once you schedule your Design Review,
it is very important to keep that appointment so others can be scheduled accordingly,
