Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Ken
Quinn, Jean Higgins, Ron Conlin, Jason Mendenhall, Ray Griffin, Merlin Miller and Eric
King.
Ben Fen'in was present to discuss his site plan.
Larry Kohntopp told Mr. Ferrin there were 3 accesses to the property, Noz Toys is required to
eliminate one access to the north and Ben Ferris is required to eliminate another access to
the north. Noz Toys and Mr. Ferrin will be required to share an access. Mr. Ferrin is
required to get a right-a-way permit from the State.
Ben Ferrin presented his landscaping proposal.
Design Review Committee approved the site plan as presented with the following condition:
1. Required to share an access with Noz Toys.
2. Landscaping requirement as presented with automatic sprinkler system to be completed
by June 30th, 2005.
3. Required to install a knox box.
4. Clear and visible address that can be seen from Yellowstone Ave.
5. Required to contain storm water drainage on site - drainage swell.
6. Address is 5060 Yellowstone Ave.
7. Required concrete, curb, gutter and sidewalk to be completed by November 1st, 2005.
8. Required to get an access permit from Idaho Transportation Department.
9. Sign on building required to be approved by Building Inspector Ray Griffin.
10. Required to get apply and get a City of Chubbuck business license.
11. Required to keep area clean.

DESIGN REVIEW' MEETING APPLICATION
The purpose of this applioation is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the Mayor, Building Official, City
Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required.
The Design Review Team meets every Tuesday morning between the hours of
10:30 and 11:30 AM.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordirmnees of the City of Chubbuek. Keep in mind, this is an
informal review and is onlyeondueted on submitted applications. This is a review team
not a design team. Provide(10) ten sets of drawings, (8 1/2 X 11), at l~st eight days in
advance ofyonr sehetl~d~ mmi=g. Formal review of your plans or project by City
Staff is conducted after formal application. Once you schedule your Design Review,
it is very important to k~p that appointment so others can be scheduled accordingly.
