Chubbuck Road Site Plan Approval Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Jean Higgins, Randy Severe, Ron Conlin, Ray Griffin, Merlin Miller, Patty Hopkins and Eric King. Vicky Stoffer and Randie Wilhelm were present to present the proposed site plan. Vicky Stoffer stated their operations are cleared by the State. Vicky Stoffer stated they do not hire any sex offenders. Vicky Stoffer said the day care facility is a regular day care facility with approx. 30 children. Vicky Stoffer said there will be a restroom installed in Space D. Upstairs will be used for storage only. Larry Kohntopp said he would like to check out the accesses, if possible there should be only two accesses, rather than three. Jean Higgins said a report showed their prior business on Hiway 30 said they denied access to Child Protection Services at one time last year. Neither one, Vicky or Randie could remember denying access to Child Protection Services. FM Merlin Miller stated there is a requirement of a knox box if approved. After discussion, Design Review Committee decided Larry Kohntopp will go to the proposed site and review the access situation. CITY OF CHUBBUCK, DESIGN REVIEW MEETING IDAHO APPLICATION The purpose of this application is to establish a meeting and review time with the City of Chubbuck Design Review Team consisting of the Mayor, Building Official, City Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review Team meets every Tuesday morning between the hours of 10:30 and 11:30 AM. This team will work in unison, with you, to establish the criteria for your project as it relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an informal review and is only conducted on submitted applicatiOns. This is a review team not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in advance of your sehefluled meeting. Formal review of your plans or project by City Staff is conducted after formal application. Once you schedule your Design Review, it is very important to keep that appointment so others can be scheduled accordingly. CENTER HOURS OF OPERATION: All children must be picked up by 6:30pmJ. f you are later than 6:30pm you will be C .~.., .ged,. $ l~r ~ minute, late fe~ payable, th~ day it occurs, We will .be closed on tho following holidays: New Years day, Thanksgiving, Christmas day, and Labor day. All other holidays wilt be d.etermined by Pm-sign up if fewer than 6 children sign up for the day we will be closed. If you sign your dfild up for the day and you do not bring your child in you Will be charg~ an ~xtra $15.00 for the day. Full-time rate for att children wilt be $ 325.00 a month part-time is $ 2.50 An hour with a minimum of 20 hours a week. After school children will be -Charged $ 2.00 an hour. Payments: Full and-part time payments are.due On the 1st oftbe month and will be considered late after the 10th. Unless Prior a~mgmeats are made. If payment has not been recieved by the 10th Of the. month you wffibe, charg a $.25.00 late. fee. ENROLLMENT~ AH paperwork must be eomplefly filled out signed; dated and turned in prior To or on child's first day. We need a copy of Immunization record for your Child/children within lstweek of attendance. An initial $50.00 payment is requir~ at_the time of~rollment txa' e/fild. Please make.sure .to update Information if any changes should oeeure. You must give a two-week written Notice .before withdm~ your child. If your child is absent more than 5 days in a row without a call.w~ will assume that he/she is no longer attending and we Will fill the.spot. You will .be charged wheather your child attends or not. However we understand-th~ people take vacation time so you will be allowed 2 weeks a year just let us know when they will be. To be eligible for the monthly childcare Families must meet income guidelines and need childeare to work or obtain an Educ~on. We understand that people forget so your allowed 2 no calls per year. Personal property: We prefer that your child does not bring any toys to the center However if they do bring a toy we will' not be held' responsible for lost or damaged Toys. DISCIPLINE: Time out and saying your sorry are types of discipline that will be Used 1 rain per age. Please feel free to visit our center any time you would like. Also remember aecidems -ha~ so please-bring an-extra-change ~of-clothes for Your child.. H~ALTH POLICY: .Sick children will not .be allowed .to att_,md our center..this includes children Who have a runny nose that is not dear, diarreah, conjunctivitis ( Pink Eye ), vomiting, or have a tempermre of 101 degrees or more Or any other contagious illness. If your child has any of these illnesses Please do not.send them to the ceater~ If your child has any of these Symptoms you wilt be called'to pick them up.. SECURITY: No child will be released to anyone .other than the parent without prior written Permission. Notes will not be accepted, from anyone. We are here for your child's safety, also if someone else will be picking up your child/children they need to Show LD. If you are a single parent and don't want your childs other parent Picking up your child, we must have a copy ora court .order on file saying 'They cannot, or a legal document showing you have sole custody, without Any paperwork we cannot refuse to let a parent pick up his/her own child.
