PURPOSE OF REVIEW: Site Plan for three buildings.
Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Merlin Miller, Larry
Kohntopp, Jean Higgins, Randy Severe, Ron Conlin, Ray Griffin and Eric King.
Design Review Committee reviewed the site plan.
Mike Johnson, MAJ Holdings was present to present the site plan.
Design Review Committee approved the MAJ Holdings site plan, subject to the following
conditions:
1. Stormwater runoff is required to be contained on site. Pre-treatment issues need
to be addressed. Required to work with Larry Kohntopp.
2. Required to have 5% interior landscaping.
3. Access is required to be 150' from intersection.
4. All tenants are required to apply for a business license.
5. Fire Department required MAJ Holdings to notify them of type of occupancy
of the buildings when they find out.
6. Required to install knox boxes, either one for all or one on each building.
7. Required to have one address, with suite letters.
8. Required to have 10' landscaping with automatic sprinkler system.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the Mayor, Building Official, City
Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required.
The Design Review Team meets every Tuesday morning between the hours of
10:30 and 11:30 AM.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuek. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in
advance of your scheduled meeting. Formal review of your plans or project by City
Staff is conducted after formal application. Once you schedule your Design Review,
it is very important to keep that appointment so others can be scheduled accordingly.
