Site Plan Approval.
Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Jean Higgins, Ken
Quinn, Ray Griffin, Eric King, Ron Conlin and Larry Kohntopp.
Corey Snow presented his site plan for a dental office at 5209 Yellowstone Ave.
Corey Snow was told he is required to move his requested access to accommodate a joint access
to the north of his property. Mr. Snow said he has talked to the State, Mr. Snow wants to
make sure the State does not have a problem with him having a joint access because they
said that since he would not have much traffic being a dental office, they will allow. City
can talk to the State if this is a problem.
Mr. Snow was advised he is required to apply for a short plat, which does not have to go before
the Land Use & Development Commission.
Mr. Snow was advised he will be assessed his portion of the street frontage improvements.
Discussion about approx costs.
Design Review Committee approved the site plan subject to the following conditions:
1. Required to have a joint access with property owner to north.
2. Required to apply for a short plat.
3. Required to contain storm water on his site.
4. Required to pay his share of street frontage improvements, which includes,
additional lane for Yellowstone Ave, curb, gutter and sidewalk.
5. 10' Landscaping with automatic sprinkling.
6. Address clearly seen from Yellowstone Ave.
7. Required to install a knox box.
8. Required off street parking, as determined by City Code.
9. Fire Hydrant to meet City Code requirements.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.
