DESIGN REVIEW SUBJECT: Conversion of 4854 Yellowstone Ave. to a Dental Office.
DATE: February W., 1999
APPLICANT: William Parrish,
PROJECT: Building just south of Ray's Custom Interiors.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steven Smart, Cary Campbell,
Jerry Rowland, Eric King, Ron C. Conlin, Jim Phelps, Larry Kohntopp and
Gerd Dixon.
Serio Romero and William Parrish, were present to present a site plan showing the conversion of
4854 Yellowstone Ave. from a residence to a Dental Office. The residence will be
reconstructed by Serio Romero for William Parrish.
Cary told Mr. Romero and Mr. Parrish the interior is required to meet building code.
William Parrish said, he plans to install a solid fence between the proposed dental office, and the
garage behind the structure.
The following was discussed and required to be presented on a site plan to Staff -
1. Required to have a minimum of 10' landscaping on Yellowstone Ave., including
automated sprinkling system.
2. Mr. Parrish is allowed a temporary access, until such time property to south develops,
at which time Mr. Parrish will be required to construct a shared radius approach. Mr.
Parrish is required to enter into a temporary access agreement with the City of
chubbuck.
3. Mr. Parrish was told a sign is required to be on private property, not taller than 3', not
shorter than 10'.
4. Mr. Parrish was told he is required to address storm drainage requirements. Mr. Parrish
will work with City Engineer on storm drainage issue.
5. Mr. Parrish was told the solid fence between the garage, and the proposed dental office
is required to be installed prior to the dental office opening.
6. BO Campbell told Mr. Parrish, the access ramp for handicap entrance is required to be
installed prior to the proposed dental office opening.
7. Parking requirement is 7 or 8 spaces.
