PROJECT TITLE: Chubbuck Chiropractic, 224 E. Chubbuck Road
PURPOSE FOR REVIEW: Residential building changed to office/clinic usage.
Present: Mayor John 0. Cotant, Attorney Tom Holmes, Steve Smart, Jerry Rowland, Ray Griffin,
Ron Conlin, Jim Phelps and Larry Kohntopp.
Dr. Jeremy Hale was present to present a site plan of his proposed Chiropractic Office, 224 E.
Chubbuck Rd. (west of Geraldine's Bakery).
Dr. Hale said he plans to install a fence to hide the garage on the property, which is not in good
shape at this time. In the future he wants to add more rooms to the back of the house and
get rid of the garage completely.
The proposed site plan was approved subject to;
1. Dr. Jeremy Hale was told the 10' landscaping with automatic sprinkling is required.
2. Parking requirement is 9'X 20'paving.
3. Dr. Hale said he would like to come to Design Review for a sign review the 9' of
August, 2000.

DEVELOPMENT REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Development Review Team consisting of the: Building Official, City Planner,
City Engineer, Fire Marshal and other City staff as required. The Development Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (8) eight sets of drawings at least one week in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.

Approval of Kit Courtyard Plan at 4708 Yellowstone Ave.
Present: Mayor John 0. Cotant, Attorney Tom Holmes, Steve Smart, Jerry Rowland, Ray Griffin,
Ron Conlin, Jim Phelps and Larry Kohntopp.
Donn Fenner and his Attorney Steve Richards were present to present to discuss the access
guidelines put on them at the June 21', 2000 Design Review Meeting,
Mr. Fenner stated he cannot do without the two accesses on to his property at this time. Mr.
Fenner felt the property will lose value if there is only one access into the property. Mr.
Fenner said Kit Manufacturing will not lease the property if there is only one access.
After considerable discussion, Attorney Holmes suggested a temporary second access ending
when the property sells or upon termination of Kit Manufacturing, Inc's lease.
Steve Smart explained the Access Management Guidelines adopted by the City Council.
The following were the requirements put on by the Design Review Meeting of June 2 l't, 2000:
1. PWD Smart stated one radius turn cut is required, maximum of 40'.
2. The production homes on Yellowstone Ave. should be grassed with automatic
sprinklers beyond the office building.
3. FD Merlin Miller stated there needs to be access for emergency vehicles.
4. Fire Department requires address numbers to visible.
5. Office building needs to be anchored.
6. Mr. Klingner said he will clean up the structure on the property.
Donn Fenner said he will talk to Kit Manufacturing about the temporary access, also if they are
willing to pay for the requirement of a radius turn cut, maximum 40', instead of the
existing curb cut.
