PURPOSE FOR REVIEW: Determine off-street parking requirements.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steve Smart, Richard Webb, Ray
Griffin, Eric King, Ron Conlin, Jim Phelps and Larry Kohntopp.
Jeanene Otto presented a site plan showing how she plans to run the Preschool.
Ms. Otto said she will have a maximum of 12 children at any one time. Ms. Otto wants to open
in June.
The following concerns were discussed:
Employee off street parking requirements
Paved lane to off street parking.
Required landscaping with automatic sprinkler system.
Handicap ramp
Fire Department is requiring a floor plan and required fire extinguishers
Smoke detectors are required
Play area needs to be enclosed.
Large letter address on front of building
Jim Phelps stated he wanted to check to make sure the area is cleaned up as City requires.
After discussion, it was decided the site plan is approved subject to Staff going to the proposed
address and advising Jeanene Otto what is required and Jeanene Otto completing the
requirements.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.

PROJECT TITLE: Store Site
LOCATION: 4888 Valenty Road
PURPOSE FOR REVIEW: Determination of requirements.
Present: Mayor John 0. Cotant, Attorney Tom Holmes, Steve Smart, Richard Webb, Ray Griffin,
Eric King, Ron Conlin, Jim Phelps and Larry Kohntopp.
Paul Lineberry, Tru -Green Chemical was present to present a building floor plan for Tru -Green
Chemical.
Fire Chief Eric King stated, with the chemicals Tru -Green Chemical is storing they will be
required to install a knox box on the front of their building. FC King explained how the
knox box works.
Fire Chief Eric King said the City requires a readable address on the front of the building.
Mr. Lineberry was told the building is required to be handicap accessible.
The site plan was approved subject to the following:
1. Install a knox box.
2. Place readable address on front of building.
3. Required to be handicap accessible.
