PURPOSE FOR REVIEW: Discussion of the proposed building use and lot.
Present: Mayor John 0. Cotant, Attorney Tom J. Holmes, Steve Smart, Richard Webb, Ray
Griffin, Merlin Miller, Ron Conlin, Jim Phelps and Larry Kohntopp.
Larry Pollard, Starr Corporation stated the business is a pharmaceutical mail order company, they
want to start business as soon as possible. Mr. Pollard said the building will be used
mainly as an office building.
Larry Kohntopp told Mr. Pollard the City needs to see the floor drain and sewer layout.
Merlin Miller requested a finished blue print on an 8 Y2x I I sheet.
Larry Kohntopp stated the site plan shows the required landscaping. Mr. Pollard was told what
the tree requirements are.
Sean needs to review the storm water retention plan.
The site plan was approved subject to the following:
1. Required to present a floor drain and sewer layout.
2. Required to fumish Fire Department with 8 Y2x I I blue print of building.
3. 10' landscaping requirement with automatic sprinkling.
4. Sean Harris needs to review and approve storm water retention plan.
Required to put address on building.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.

APPLICANT: Kim Kunz, 4855 Galena, Chubbuck
PROJECT TITLE: Automotive Lube/repair shop
LOCATION: 4950 Yellowstone Ave. (RV Service and Supply Bldg).
PURPOSE FOR REVIEW: Discussion of the proposed building use.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steve Smart, Richard Webb, Ray
Griffin, Merlin Miller, Ron Conlin, Jim Phelps and Larry Kohntopp.
Kim Kunz presented has site plan along with his plans.
Larry stated there is no realistic way to install landscaping at this time.
Mr. Kunz stated the northwest door will be used as the people entrance.
The following was discussed and required for approval of the site plan:
1. Vehicles kept overnight are required to be inside the shop.
2. No parking on the north side of building on Ward Drive.
3. Sign will be installed on building, if a free standing sign is installed, it will be required
to go before Design Review Committee.
4. Floor drain is required to be plugged.
5. Kim Kunz is required to enter into a non residential waste water agreement with the
city.
6. Ray Griffin will check on size of door, Mr. Kunz might be required to install another
door.
7. Fire Department is requiring fire extinguishers located as Code requires.
8. No parking on the northwest side of the building so as to interfere with the site triangle
of Yellowstone Ave and Ward Drive.
