Holmes, Steve Smart, Merlin Miller, Ray Griffin, Richard Webb, Ron Conlin, Eric King, Jim Phelps and Larry Kohntopp. Mayor Cotant stated, because he is current owner of property at 4806 and 4836 Hawthorne Road he will excuse himself from the meeting. Jim Shaddack presented a site plan for a proposed assisted living care center located at 4806 and 4836 Hawthorne Road. Mr. Shaddack asked, because of there being only two employees the Design Review Committee require only 6 parking spaces. The only other parking will be visitors. Mr. Shaddack stated the center will be fenced off, and will have a fire sprinkling system. The following was discussed and required: I - Jim Phelps asked that their property to the east include a ground cover and be maintained. 2. Sidewalk, curb and gutter required. 3. Required to apply for a short plat so the 15'right-a-way dedication is taken care of 4. Have required setbacks. 5. Fire hydrant on their development side of street is required. 6. Apply for a Conditional Use Permit because of the C- I zoning. 7. Storm water drainage required to be on site. 8. 10' Landscaping required with automatic sprinkling. 9. Required to provide paving from 15' curb and gutter to street. 10. Knox box required. I I - Visible address on front of assisted living center. 12. Merlin Miller and Ray Griffin will review their plans after this meeting. 13. Six parking spaces are required. "MW CITY OF CHUBBUCK, IDAHO DESIGN REVIEW MEETING APPLICATION The purpose of this application is to establish a meeting and review time with the City of Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon. This team will work in unison, with you, to establish the criteria for your project as it relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an informal review and is only conducted on submitted applications. This is a review team not a design team. Provide (6) six sets of drawings at least eight days in advance of your scheduled meeting. Formal reviews of your plans or project by City staff is conducted after formal application. Once you make your appointment and it is verified it is very important to keep that appointment so others can be scheduled accordingly.
