PURPOSE OF REVIEW: Site Plan approval.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steve Smart, Ray Griffin, Merlin
Miller, Jason Mendenhall, Ron C. Conlin, Jim Phelps and Larry Kohntopp.
Larry Kohntopp stated Bill Orr wants to operate a use car lot at 4612/4818 Yellowstone Ave.
Mr. Orr stated there will be no vehicle rebuilding at this address.
Larry Kohntopp said the landscaping needs to be revived because no one has kept up, two trees
need to be planted.
Discussion about parking requirements.
Larry said he told Mr. Orr, if he decided to bring his production building on the site for a sales
office, it would be required to have a permanent foundation.
Larry said Mr. Orr stated he is attaching a sign to the sales office.
Design Review Committee approved the application subject to the following conditions:
1. No rebuilding wrecked vehicles at the 4612/4618 Yellowstone Ave. property.
2. 4612/4618 Yellowstone Ave will be used as a used auto sales lot only.
3. Landscaping with automatic sprinkling is required to be brought to City Code.
4. Two each, 1 1/2" trees are required to be placed in the landscaping.
5. Mr. Orr is using the building on site for a sales office, if a production building is used
on the site, it is required to be put on a permanent foundation as required by City Code.
6. If a sign is installed other than on the sale office building Mr. Orr is required to present
the plan to the Design Review Committee.
7. Provide Mr. OrT with City of Chubbuck tree list.

The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.

PROJECT TITLE: Physical Therapy Business and Church
LOCATION: UPPR/Siphon Road
PURPOSE OF REVIEW: Site approval.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steve Smart, Richard Webb, Ray
Griffin, Eric King, Merlin Miller, Ron Conlin, Jim Phelps, Jason Mendenhall and Larry
Kohntopp.
Sharik Peck was present to explain his plans with the property he purchased.
Mr. Peck Mr. Robinson were told they are required to plat the property.
Discussion about street approach, curb, gutter and sidewalk, all weather access for the Fire
Department and landscaping with automatic sprinkling.
Race Robinson, Christ's Love Vineyard Church was present and is using a portion of Mr. Peck's
property. Mr. Robinson said he would like to start on the church building right away.
Both Mr. Peck and Mr. Robinson said they could not afford to complete the curb, gutter and
sidewalk as required.
Discussion about grassing the property from the street to a paved walkway through the front of
the property.
Mayor Cotant stated he did not have a problem waiving the sidewalk requirement, while
allowing a walkway meandering through the front of the property.
After discussion, Design Review Committee approved the application with the following
conditions:
I - Curb, gutter, sidewalk and landscaping with automatic sprinkling is required on
property facing Whitaker Road.
2. Waive the sidewalk requirement while allowing them to grass from curb and gutter to
paved sidewalk and paved parking.
3. Required to have automatic sprinkling system for grass area.
4. Fire Marshal Merlin Miller stated they are required to contact Fire Department about
fire hydrant placement
5. Required to pave a walkway in place of sidewalk between the grass and paved parking.

6. Need required emergency vehicle access. Will work with Fire Department.
7. Address on buildings, need to be clearly visible from Siphon Road.
8. Need to apply for plat along with applying for a conditional use permit for the church.
9. Required to have a shared, "street -type" approach on Siphon Road.

PURPOSE OF REVIEW: Determine requirements for new commercial business.

Florence Miller was present to show her plan for landscaping at 5039 Yellowstone Ave,
(attached).
Mrs. Miller said, she plans on using the existing sign the laundromat used, in the future, but for
now she is attaching a sign to the building. Mrs. Miller will contact the City when she
uses the sign out front.
Merlin Miller said the Fire Department requires an address be on the front of the building. Mrs.
Miller said she will make sure there is a readable address on the front of the building.
Design Review Committee approved the Design Review Application and site plan as presented.
