PURPOSE OF REVIEW: Business and site plan approval for health care equipment
and supplies, also oxygen.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Richard Webb, Ray Griffin, Eric
King, Ron Conlin and Jim Phelps.
Becky Hancock, APREA Health Care was present to present her site plan.
Becky Hancock said the APREA Health Care used to be Medical Mart, they sell health care
equipment and supplies, including oxygen. Becky said they have six employees. Hours
are 8:30 a.m to 5:30 p.m. There are no walk in customers, just delivery.
Becky was told she is required to have paved parking for the employees. She said there is
graveled parking in the back for the employees.
Becky Hancock stated she received a letter from the Chubbuck Fire Department advising her she
is required to have a knox box and a clear address on the front of the building.
Becky stated the owner of the building said he would do something with the grass in front for
additional parking, Becky Hancock was told the building is required to have landscaping
with automatic sprinkling, so it is doubtful the grass area can be used as parking.
Becky Hancock said a sign will be attached to the building.
The site plan was approved subject to the following conditions:
1. Paved drive and parking for employees.
2. 10' Landscaping area with automatic sprinkling system.
3. Knox box required.
4. Clear address on front of building.
5. Required to work with Fire Department in having a vault.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified it is very
important to keep that appointment so others can be scheduled accordingly.
