Present: Mayor John O. Cotant, Attorney Thomas J. Holmes, Steve Smart, Richard Webb,
Merlin Miller, Ray Griffin, Eric King, Ron Conlin, Jim Phelps, Larry Kohntopp and
Councilman Mary Gunter.
Dave Orthel was present to present his site plan.
Larry Kohntopp advised Mr. Orthel he is required to share an access with property to south and
property to the north at such time the property develops. Dave Orthel said he will sign a
shared access agreement.
Mr. Orthel said he will still want to construct a twenty foot street with radius type approach to the
property.
Discussion about fire hydrant requirement, Merlin Miller said the hydrant will have to be within
400'.
Discussion about requiring a knox box, after discussion it was decided a knox box is an option.
Steve Smart stated the City purchased some right -a -way he suggested Mr. Orthel make sure there
is enough set back space.
After review of the site plan the Design Review Committee approved the site plan with the
following conditions:
1. Required to enter into a shared access agreement with property to south now,
and north depending what develops to the north.
2. Street type approach with a paved drive and paved parking.
3. Address is required to be visible from the street.
4. 10' landscaping with automatic sprinkling system.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.
