PURPOSE FOR REVIEW: Request approval to convert a residence to an Insurance
Office.
Present: Mayor John 0. Cotant, Attorney Thomas J. Holmes, Steve Smart, Richard Webb, Ray
Griffin, Eric King, Merlin Miller, Ron Conlin, Jean Higgins and Larry Kohntopp.
Rick Olson was present to present his site plan for an insurance office at 121 Evans Lane.
Mr. Olson stated the employee parking in front of the garage is all paved.
Mr. Olson stated he is hiring a surveyor.
Design Review Committee approved the site plan subject to the following requirements;
1. Landscaping with automatic sprinklers by June I ", 2004.
2. Handicap requirements.
3. Fire Department required a knox box.
4. Be able to see clear address from street.
5. Chubbuck Building or Fire Inspector will do a walk through.
6. Required to manage storm water run off to the south.
7. Steve Smart will check to see if the proposal should be platted.

DESIGN REVIEW MEETING AP)L17ATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the: Mayor, Building Official, City Planner,
City Engineer, Fire Marshal, Police Chief and other City staff as reqw'red. The Design Review
Team meets every Tuesday morning between the hours of 10:30 and 12:00 Noon.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on submitted applications. This is a review team
not a design team. Provide (6) six sets of drawings at least eight days in advance of your
scheduled meeting. Formal reviews of your plans or project by City staff is conducted
after formal application. Once you make your appointment and it is verified, it is very
important to keep that appointment so others can be scheduled accordingly.

The attached overhead site plan is a rough draft. We are
open to Suggestions from the City Of Chubbuck. This site
plan shows where:
1. 3 Parking spaces(one handicapped sp)
2. Location of an electric two sided sign
3. Location of the handicapped ramp
Rick Olson (Olson Insurance) would like to put in
an insurance office at 121 Evans Lane in Chubbuck.
The proposed move into the Office would be mid to late
January 2004.
At present this building is used as a two bedroom home with
storage space down stairs. 800 square feet is the
approximate area on the main floor.
This lot is directly East of Subway Sandwiches in Chubbuck.
We have been operating an insurance Office since 1986 at
4544 Yellowstone Ave , Chubbuck, Id. Our lease is expiring
on Feb 1 2004.
