DESIGN REVIEW COMMITTEE MEETING MINUTES September 19", 2006 APPLICANT: PROJECT TITLE: LOCATION: PURPOSE OF REVIEW Arby's (Jared/Harper-Leavitt Engineering). Arby's Restaurant 4501 Yellowstone Ave. Site Plan approval Present: Mayor Steven M. England, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Ron Conlin, Randy Severe, Ron Osborn, Merlin Miller and Eric King. Jared Bragg, Harper -Leavitt Engineering presented a proposed site plan for Arby's Restaurant, 4501 Yellowstone Ave. Mr. Bragg was advised Idaho Transportation Department will require permits for working on Yellowstone Ave. Discussion about where they will hook to water and sewer services. PWD Smart stated the existing water service is probably 3/4", they will need 1" service. The service will come from Yellowstone Ave. After review, Design Review Committee approved the site plan with the following conditions: 1. Arby's sign is required to be setback from the sidewalk right-of-way. 2. Fire hydrant is required to be updated and to be out of sidewalk right-of-way. 3. Burnside Ave approach is required to be a radius type approach. 4. Minimum 1500 gallon grease interceptor is required. 5. 10' landscaping with automatic sprinkling is required on Yellowstone Ave, and also Burnside Ave, between approaches. 6. Storm water is required to be contained on site. 7. Knox Box is required. 8. Required to get the necessary demolition permits for the existing structures on property. 9. Arby's will work with Wendy regarding their cross access agreement. 10. Required State setbacks on Yellowstone Ave. CITY OF CHUBBUCK, IDAHO DESIGN REVIEW MEETING APPLICATION The purpose of this application is to establish a meeting and review time with the City of Chubbuck Design Review Team consisting of the Mayor, Building Official, City Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required. Contractor shall not willfully proceed with construction as designed when it is obvious that unknown obstructions, area discrepancies and/or grade differences exist that may not have been known during design. Such conditions shall be immediately brought to the attention of the Owners Authorized Representative. The contractor shall assume full responsibility for all necessary revisions due to failure to give such notifications. 2. The contractor shall follow the landscape pion. Any discrepancies between the plan and field conditions shall be reported to the Owner's Authorized Representative before proceeding with work. 3. Contractor shall be responsible for any coordination with subcontractors as required to accomplish all construction operations. All piping, conduit, sleeves, etc., shop be set in place prior to installation of construction items. 4. All existing improvements, to remain shall be properly and adequately protected from damage during construction operations. It shall be the responsibility of the Contractor to restore to the original condition any existing items that are damaged or disturbed in any way. & If there is o discrepancy between the plant counts on the call -outs on the plans & the actual number of plant symbols depicted on the plans, the contractor is responsible for installing the actual number of plants as symbolically depicted on the plans. 6. Contractor to contact the Utility Notification Center of Idaho before digging, including but not limited to, trenching AND shrub and tree planting pits. If utilities occur at locations of proposed shrubs, or within eight (8) feet of proposed trees, the contractor shall report such conditions to the Owner's Representative. Damage to existing utilities by the Landscape Contractor is the responsibility of the Landscape Contractor. 7. All shrub beds/cobble areas to be contained 4" green, Nei' thick Pro Steel edging (or approved equal) as per plans. 8. All shrub beds will be mulched with 1 1/2" washed river rock at a minimum of 3 inches deep over Iandscgape/weed barrier fabric. 9. Install Fir Fiber mulch ring around trees and shrubs in turf areas as shown on planting details below. 10. Sod areas shall receive a minimum of 4 Cu. Yds per 1000 S.F. of Premium Organic compost, or o well decomposed compost approved by the Owner's Authorized Representative. Shrub beds shall receive a minimum of 4 Cu. Yds. per 1000 S.F. of one of the above materiels. All amendments shall be thoroughly tilled 6" into the soil. 11. Landscape Contractor to provide finish grades while maintaining positive drainage. Contractor to notify Owner's Representative of any poorly drained areas prior to commencing work. 12. All trees, shrubs, and other plant materials are to be approved by the Owner's Rep. prior to delivery and again after delivery to site. Any plant not meeting approval may be rejected at any time prior to Final Acceptance. 13. All plant material locations are to be staked by the contractor for review by the Owner's Rep. All adjustments shall be made by the contractor. The contractor shall not dig plant pits until locations are approved. 14. Landscape Contractor to provide staging plan and schedule for completion to the Owner's Rep. prior to mobilization. 15. Cobble areas to receive washed 3-6" River cobble placed over landscape filler fabric. 16.
