DESIGN REVIEW COMMITTEE MEETING October 16"', 2007 APPLICANT: PROJECT TITLE LOCATION: PROPOSE OF REVIEW Brock Wilkes, Doug Briscoe/Dioptra Geomatics Mortuary 150 Block of W. Chubbuck/4700 Block of Afton Place. Site plan approval. Present: Mayor Steven M. England, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Ron C. Conlin, Randy Severe, Ray Griffin, Merlin Miller and Eric King. Doug Briscoe and Brock Wilkes presented the site plan. Design Review Committee approved the site plan with the following conditions: 1. Sign or marquee is required to be approved by a City Building Official. 2. Comply with landscaping requirement with automatic sprinkler system. 3. Clear address on building. 4. Accesses shall be 150' from intersections. 5. Storm water drainage to be contained on site. Work with City Staff on a plan. 6. Mr. Briscoe stated they will install a sand & water interceptor in a garage. Required to install a knox box. 8. Fire sprinkler installation shall be inspected by an inspector. 9. Any chemicals shall to be stored away from sewer. 10. Required to present a set of plans to Fire Department. Might need to have more hydrants. 11. Required to have a dumpster pad enclosed on three sides. 12. Required to present a digital copy of site plan to City. 13. Required to enter into a Development Agreement with the City. 14. Access shall be Afton Ave to W. Linden Ave to Yellowstone Ave. 15. There will be no Occupancy Permit issued until all requirements are completed. 16. Development Agreement will require a guarantee bond. CITY OF CHUBBUCK, IDAHO DESIGN REVIEW MEETING APPLICATION The purpose of this application is to establish a meeting and review time with the City of Chubbuck Design Review Team consisting of the Mayor, Building Official, City Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review Team meets every Tuesday morning between the hours of 10:30 and 11:30 AM. This team will work in unison, with you, to establish the criteria for your project as it relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an informal review and is only conducted on Submitted applications. This is a review team not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in advance of your scheduled meeting. Formal review of your plans or project by City Staff is conducted after formal application. Once you schedule your Design Review, it is very important to keep that appointment so others can be scheduled accordingly.
