Site Plan Approval. Present: Mayor Steven M. England, Councilman Kermit Morrison, Attorney Thomas Holmes, Steve Smart, Larry Kohntopp, Ron Conlin, Randy Severe, Ray Griffin, and Andy Despain. Doug Briscoe, Mark Kinghorn and Owners presented the revised site plan. Mark Kinghorn stated he checked with City of Pocatello and there is not a grease trap requirement on this size of operation. Mr. Kinghorn said they are still checking on the fire sprinkler system requirement. After discussion the assisted living center decided to use 4 sanitation containers per building. The revised site plan is approved with the conditions in the April 28", 2008 Design Review Committee Meeting Minutes, along with these conditions: 1. Required to follow Fire Sprinkler System requirements as per Fire Department. 2. They are not required to have a grease trap separator with this operation. 3. Present: Mayor Steven M. England, Councilman Kermit Morrison, Attorney Thomas J. Holmes, Steve Smart, Larry Kohntopp, Ron Conlin, Randy Severe, Ray Griffin, Eric King, Merlin Miller and Andy Despain. Doug Briscoe and Mark Kinghorn presented the site plan. Mr. Briscoe stated they want to build the first building and then start with the next one when finished with the first. It was stated there will a maximum of 15 residents per building. There will be no resident vehicles. Discussion about the common areas for both buildings. The following was discussed: 1. They were asked to move the 24' road access approx 5' to the east. Discussion about patient drop off in the access. 2. Discussion about off street parking - Should be one space for two beds. 3. Larry Kohntopp recommended they reconfigure the buildings. They said they considered this, but, there are transformers on the property they did not know about. 4. They said there will be a walk ramp. 5. They said they will most likely use a monument type sign. They will show Building Department plans when a decision is made. 6. Steve recommended they use a scupper that goes under the sidewalk for storm water drainage. 7. Larry Kohntopp said he will check with Pocatello to see what they require for grease - oil separation for this type of business. 8. Exterior lighting is required to be contained on site. Shielded in some way. 9. Merlin Miller said if they have a code pad system and Fire Department knows the code, they will not be required to have a knox box. 10. Merlin Miller asked if he could get the building sprinkling plans. 11. Ray Griffin said when the sanitation carts will not work for them, they are required to have a 3 sided enclosure for a dumpster. 12. Required to have 10' landscaping with automatic sprinkling system. DESIGN REVIEW COMMITTEE MEETING MINUTES - (cont.) April 29", 2008 The representatives told the City would like to see the revised plans showing all changes. After discussion, it was decided the Design Review Committee Meeting will be continued on May 6"', 2008 at 10:30 a.m. CITY OF CHUBBUCK, IDAHO DESIGN REVIEW MEETING APPLICATION The purpose of this application is to establish a meeting and review time with the City of Chubbuck Design Review Team consisting of the Mayor, Building Official, City Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required. The Design Review Team meets every Tuesday morning between the hours of 10:30 and 11:30 AM. This team will work in unison, with you, to establish the criteria for your project as it relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an informal review and is only conducted on Submitted applications. This is a review team not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in advance of your scheduled meeting. Formal review of your plans or project by City Staff is conducted after formal application. Once you schedule your Design Review, it is very important to keep that appointment so others can be scheduled accordingly. Proposed Project: Build two Assisted Living Facilities— Licensed for fifteen residents in each facility. Proposed Location: Lot 1 and 2 of Block 1...A replat of lot 4 Block 1 Pheasant Ridge Townhouses phase 2. The property is located behind the Sinclair gas station on Hiline between Alamo Street and Pheasant Ridge Drive. IITEMS TO BE DISCUSSED: Traffic Safety: a. The commercial property has been purchased to accommodate increased traffic flow due to visitors and health care professionals. The majority of traffic would not be using residential streets to come and go from the facilities. b. In addition, the site will have off-street parking and will comfortably accommodate staff and visitors. 2. Outdoor Advertising: a. The owners of the proposed facilities would like to have their signage be a large landscape rock, one for each facility, with their company name etched in the rock. It would be assumed that the rocks would be moderate in size, not huge obnoxious boulders. 3. Landscaping: a. The facilities would be professionally landscaped, beautifying the location, neighborhood, and community. b. The planter strip would meet city requirements with regards to the type of trees planted. The facilities landscaping would include trees, shrubs, bushes, plants, and sod. 4. Structures and Site Lay -Out: a. Refer to the Site Plan. 5. Drainage: a. A professional, for the purpose of proper layout and grading, has prepared a site plan for the proposed location. Therefore, the property will maintain ILS own storm water relative to all hard surfaces. 6. Pretreatment: Page 1 7. Exterior Lighting: a. The exterior lighting will be aesthetically attractive and non -obtrusive to adjacent residential or commercial properties. The fixtures will not be large and put off large amounts of light that will need to be shielded. 8. Fire Department: a. The exterior of the building will be equipped with a Knox box in case of an emergency. b. The developer has installed two fire hydrants, one located on Alamo Street and one located on Pheasant Ridge Drive. Both hydrants are within a close proximity to the said property. c. Each facility will be equipped with a 13-D fire sprinkling system. The 13- D system is a system that meets all state regulations with relation to the size and scope of each facility. d. The address of each facility will be placed in a location that will be visible to the public and to emergency personnel. 9. Building Department: a. A licensed professional, meeting all city code requirements, will draw the architectural plans. b. The plans will reflect all ADA requirements i.e. door openings, hand railings, toilets, counter heights etc. c. The facilities will use residential containers for garbage disposal. 10. Police Department: a. The contractor and the owners of the facilities do not foresee any safety issues relative to law enforcement.
