PROJECT TITLE: State Farm Office
LOCATION: South of Wilkes Funeral Home.
PURPOSE OF REVIEW: Site Plan Approval.
Present: Mayor Steven M. England, Councilman Kermit G. Morrison, Attorney Thomas J.
Holmes, Steve Smart, Larry Kohntopp, Ron Conlin, Randy Severe, Ray Griffin, Eric
King, Andy Despain and Merlin Miller.
Doug Briscoe and Rodney Burch were present to present the site plan for Jeff Lunt.
Mr. Burch stated these will be condominium type offices. Units A & B. Mr. Burch stated there
will be a State Farm Office and his own office.
The site plan was approved subject to the following conditions:
1. Required to complete the required landscaping with automatic sprinkling.
2. Stow' water drainage is taken care of.
3. Exterior lighting to be shielded on property.
4. A short plat is required.
5. Required to have a knox box on the buildings.

LOCATION: 4170 Hawthorne Road
PURPOSE OF REVIEW: Site Plan Approval.
Present: Mayor Steven M. England, Councilman Kermit G. Morrison, Attorney Thomas J.
Holmes, Steve Smart, Larry Kohntopp, Ron Conlin, Randy Severe, Ray Griffin, Eric
King, Andy Despain and Merlin Miller.
Josh Heilesen, Summit Builders was present to present the site plan.
Staff told Mr. Heilesen there needs to be another access other than the Hawthorne Road access.
The access problem needs to be resolved prior to the site plan being approved.
Mr. Heilesen stated these will be higher end commercial, Dental, Doctor Offices.
If access concerns can be solved, the following are the conditions of approval:
1. Need recordable easement for additional access. North/south parking could be marked
as access.
2. Hawthorne Road access shall be a 20-30' radius type street approach.
3. When installing a sign Building Official shall be advised.
4. 10' landscaping with automatic sprinkler along Hawthorne Road and Mall
Entrance Road.
5. All storm water drainage to be contained on site. The plan shall be engineered.
6. Exterior lighting shall be contained on site.
7. Required to apply for a short plat.
8. Required to have a Professional site plan, ADA requirements.
9. Required to have dumpster pad with 3 sided enclosure.
10. Required to install a knox box.
11. Need clear address on the buildings.
12. Required to show location of fire hydrant on site plan.

DESIGN REVIEW MEETING APPLICATION
The purpose of this application is to establish a meeting and review time with the City of
Chubbuck Design Review Team consisting of the Mayor, Building Official, City
Planner, City Engineer, Fire Marshal, Police Chief and other City staff as required.
The Design Review Team meets every Tuesday morning between the hours of
10:30 and 11:30 AM.
This team will work in unison, with you, to establish the criteria for your project as it
relates to the adopted ordinances of the City of Chubbuck. Keep in mind, this is an
informal review and is only conducted on Submitted applications. This is a review team
not a design team. Provide (10) ten sets of drawings, (8 1/2 X 11), at least eight days in
advance of your scheduled meeting. Formal review of your plans or project by City
Staff is conducted after formal application. Once you schedule your Design Review,
it is very important to keep that appointment so others can be scheduled accordingly.
